I certify that I have truthfully disclosed all known medical conditions. I understand that failure to disclose relevant medical information may increase risk of complications.*

I have reviewed the design, placement, spelling, and size of the tattoo. I approve it as final. I understand that tattoos are permanent and that removal is difficult, expensive, and may not be fully effective.*

I understand that the following risks are possible:

● Infection

● Allergic reactions

● Scarring

● Blowouts

● Fading

● Changes in pigment over time

● Pain, swelling, or bruising*

I voluntarily consent to the tattoo procedure.

I release and hold harmless the artist, studio, and any affiliates from any liability related to:

● Known or unknown medical complications

● Allergic reactions

● Infection due to improper aftercare

● Dissatisfaction with aesthetic outcome (if design was approved)*

I confirm that I have received and understand written and/or verbal aftercare instructions. I agree to follow those instructions.*

I understand changes to the design may require additional cost.

Touch-ups are subject to studio policy.*

I give permission for photos/videos of my tattoo to be used for marketing, social media, and portfolio purposes.

I confirm that I am not under the influence of drugs or alcohol.

I understand that body art is a physical procedure and accept full responsibility for any consequences arising from my decision.

I acknowledge that no guarantees have been made regarding the outcome.*
